Employee dies after losing consciousness

At 11:00 a.m. on January 19, 2022, an employee was operating a forklift to weigh slag pans. The employee fainted after exiting the area. He was taken to the break room, where he complained about dizziness. The employee was sitting down in a chair, acting coherently in the break room, when he started vomiting and he lost consciousness. The ambulance came and took him to the hospital, where he was pronounced dead.

At 12:45 p.m. on August 25, 2018, an employee was preparing the # 3 furnace for tapping of slag and was using a steel pry bar inserted into access holes at the top of the furnace to move any material. The power supply is turned off; however, there is some residual amperage in the furnace electrodes and the employee was observed through the surveillance video suddenly collapsing backwards with his arms and legs continuing to move as he slid down the stairs on his back. The Medical Examiners findings described skin burns on the employees hands as evidence of electrocution. Two coworkers attempted CPR and used an AED to revive the employee; however, the employee was pronounced dead after arrival at the Bayshore Regional Hospital.
