Employee is struck and killed by trailer
At 5:45 p.m. on June 15, 2015, Employee #1 was assisting with collapsing an extendable flatbed trailer. Employee #1 was standing on the passenger side of the tractor near the manual levers between the trailer headboard and flatbed extendible trailer. A coworker backed up the tractor to collapse the flatbed extendable trailer and Employee #1 was caught between the trailer headboard and flatbed trailer. Employee #1 was observed reaching in as the tractor was backed up. Employee #1 suffered abdominal contusions and was killed.
