Employee dies of cardiac arrest while monitoring machines
At 7:00 a.m. on August 11, 2021, an employee had been monitoring three rows of rice milling machines when the he was found laying on ground unresponsive. The employee had died of cardiac arrest just prior to ending his work shift.
Employee's thumb is partially amputated when caught between
At 9:49 p.m. on June 15, 2017, an employee was operating a Hyssen Form Fill Seal machine (Poly #9) that was being used to seal plastic bags filled with the rice. During operation, a plastic bag became caught in the machine's jaw plate. As the employee reached underneath the machine to free the bag, her left thumb became caught between two metal plates. As a result, the thumb was partially amputated.
