Employee amputates fingers while installing cartridge
At 1:00 p.m. on July 2, 2108, a crane operator, was lowering a catalyst cartridge into a stationary frame. The catalyst cartridge measured four feet in diameter, 21 feet in length, and approximately 38 tons. Employee #1 and #2, mechanical technicians, were at the bottom guiding the cartridge into the support ring, and were trying to align the flanges. Employee #1 put his hands between the support ring and the cartridge. When the cartridge was lowered, it crushed his fingers, amputating them. The employee was hospitalized for his injury.
