At 8:45 p.m. on November 20, 2018, an employee was working on the horizontal puller machine when he reached in and grabbed for the tubing for a roll of composite. The employee caught his hand into the machine and amputated his finger. A second employee was working with the dry wrap machine and had her hand pulled into the machine, amputating one of her fingers. Both employees were hospitalized.

At 2:30 a.m. on October 31, 2018, an employee was investigating some smoke that was coming from a 55-gallon drum full of 721 resin when a fire flared in the drum and the employee burned his face, back, arm and knee. The employee was hospitalized.
