Employee dies from cardiac arrest

At 8:30 a.m. on February 17, 2023, an employee and a coworker were draining a liquid knockout tank into drums. They had completed the task, and the employee was cleaning the area by spraying it with a water hose. The coworker went to a nearby building, but came back within approximately one to two minutes and found the employee on the ground unconscious. The EMS was called and arrived two minutes later. The employee had gone into cardiac arrest and was taken to the hospital, but died. The employee died due to hypertensive atherosclerotic cardiovascular disease (natural causes).

Employee sustains minor burns after explosion and fire

At 10:45 a.m. on July 31, 2019, Employee #1 was in the N2 analyzer house working on an analyzer machine. Employee #1 heard an explosion and looked out the window and saw debris falling. Employee #1 was told by a coworker to run when he saw their bikes on fire and then ran. Employee #1 received minor burn injuries to the back of his neck and had his eyebrows singed. He was treated but not hospitalized. The fire was caused by a NT-01 explosion.

Employee is splashed by hot oil while clearing pipeline and

At 11:00 a.m. on May 18, 2017, an employee was using a reaming tool to unplug a line. The employee attached the reaming tool to port to unplug line. When done, the employee relieved the pressure by using the bleeder on the tool. When employee removed the reaming too, he discovered that he had not closed the valve and was splashed with hot oil on his right arm and lower back. The employee received second degree burns from the hot oil.
