At 9:44 a.m. on April 2, 2021, an employee drove a company truck to a rail yard located outside the facility. The employee walked for about 100 yard before he sat down and rested and then collapsed. He pressed his radio distress button and was later found by coworkers unresponsive. The employee's mouth was foaming and clinched and he died before reaching the hospital.
